RN Coding Specialist:

To provide expertise to assigned manager and staff for ICD 9-10 coding and OASIS/485 review and compliance

Performs ICD 9-10 coding for all start of care, resumption of care, and recertification visits in an accurate and timely fashion.

  • Works in conjunction with Intake to ensure all appropriate information needed for coding is obtained in intake process.

  • Utilizes advanced, specialized knowledge to review medical record documentation (referral, H&P, discharge summary) and accurately assign diagnostic codes to 485s and related OASIS documentation, using ICD 9-10 and Home Health coding conventions.

  • Keeps current with developing issues and trends in medical coding procedures/requirements.

  • Follows mandated reporting procedures for accurate and timely completion and submission of coded data.

  • Assures coding congruencies are addressed by clinical staff according to regulatory guidelines.

Reviews OASIS documentation and Plan of Care for completeness and accuracy.

  • Reviews admission, resumption, recertification, transfer, and discharge OASIS for omissions and errors and inconsistencies.

  • Completes reviews in a timely fashion as defined by leadership.

  • Discusses any omissions or inconsistencies with clinician to resolution.

  • Provides education and direction to clinician on correct procedures.

  • Assures Plan of Care is reflective of and appropriate to the assessment.

  • Works one on one with assigned staff to monitor appropriate utilization of resources based on the HHRG, Plan of Care and patient outcomes.

  • Performs insurance ongoing visit authorizations.

  • Participates in regulatory surveys as needed.

  • Participates in ADR review and audits charts as appropriate.

Facilitates performance improvement initiatives by providing data that identifies the success or failure of performance improvement initiatives.

  • Implements clinical monitoring activities in accordance with agency performance improvement plans (i.e. quarterly audits, surveillance chart audits).

  • Assists in identification and implementation of appropriate measures of correction of identified problems or concerns in the delivery of home care.

  • Identifies opportunities for improvement and educates staff to ensure documentation is thorough and correct.

  • Prepares and submits written quality summary reports for the assigned manager and the Home Health Leadership team.

Provides care coordination with patients and care providers.

  • Triages patient phone calls determining needs of patients with goal to provide quality care and reduce the need for emergency room visits or hospitalizations. Define if extra nursing visit should be made.

  • Keeps assigned manager informed of patient clinical progress or problems.

  • Receives physician phone calls with patient orders.

  • Notifies appropriate nurse of order.

  • Notifies nurse and/or patient of any change in plan of care as ordered by the physician.

  • Documents all conversations with the client, physician and/or physician order in the patients’ EMR.


  • Bachelor's Degree in the field(s) of Nursing or Health related field required.

Work Experience

  • 3-5 years of experience in/with acute nursing background required

  • 3-5 years of experience in/with home health required.

  • Past leadership experience preferred.

License and Certification

  • CPR

  • Registered Nurse in the state of IL

  • Other: OASIS Certification within 3 years.

  • Other: ICD 9-10 certification within 3 years.
