Our clinical team is seeking a Team Care Coordinator, who will be located at our Lombard, IL office, will be the first impression person for all the members of the Interdisciplinary Teams and responsible for performing diversified and confidential receptionist and administrative support duties for the IDG. This position reports to the designated Team Leader and is responsible for coordinating and managing the overall non-clinical processes and systems in order to support the clinical team to provide excellent patient care. Supports communication between IDG team members, patients and families and all JourneyCare programs as appropriate.

Monday through Friday 8:00am to 5:00pm day shift position

  • Answers intake phone calls and provides information regarding the agency and collects, processes and records information regarding patient referrals and admissions.

  • Creates and coordinates the preparation of reports, analyzes data, and identifies solutions

  • Prepares and edits correspondence, presentations and reports using appropriate software and tools

  • Assembles check requisitions, expense reports and other statements for signature

  • Maintains and manages filing system (physical and electronic) and storage of documentation

  • Supports department with typing, photocopying, faxing and mail distribution

Required Experience:

  • High School Diploma

  • Three plus years administrative experience

  • Intermediate Computer skills (Word, Excel, Power Point, databases, e-mail)

  • Verbal and Written Communication Skills

  • Organizational and Customer Service Skills
