Essential Functions of the position are detailed below, and include any physical requirements below that.1. Works independently and consistently exercises discretion and sound judgment in the area of case management. Recognizes problems and provides for best practice solutions, with supervisory consultation, as needed. Provides comprehensive risk assessments and medical case management services to approximately 60 high risk pregnant women and post-partum women in compliance with program plan and contractual agreements with funder. Conducts all services in clients' home.2. Completes previous and current health information of each client and is responsible for accurate and timely input of required data for the prescribed data system.3. Engages the client in formulating care plans by mutually identifying target problems, goals and tasks. 4. Identifies and coordinates medical and social services to ensure best- possible outcomes for each pregnancy. Provides linkage and advocacy for social services as indicated by the client assessment as well as coordination of service care with client's primary care physician, obstetrical care, and any other appropriate specialty care.5. Assess the needs of clients and provides linkage and advocacy to ensure the client is able to access the needed services. Identifies with the client a medical provider. Provides prenatal health education, referral for childbirth education and parenting classes. Provide referrals for the infant and mother to the Family Case Management services six weeks postpartum. Ensure all clients are enrolled in the Women and Infant Food Program (WIC) and establish and maintain a medical provider.6. Consults with and reviews client records with supervisor and Quality Assurance Nurse around issues and service provision. As a mandated reporter, immediately notifies the supervisor/director and the Department of Children and Family Services (DCFS) of any suspicions and findings of abuse or neglect.7. May provide orientation, training and/or supervision to student interns or to new employees in the family case management program. May accompany co-workers on home visits.
